OUR CONTRACT FOR SERVICES
All patients starting our program are required to sign an agreement for services. Patients who are doing well, in good appetite suppression, and losing good weight come into the clinic every week. The cost of the program is $50.00 for the initial consultation (initial consultation amount is waived if you start the program that day) and $15.00 per weekly office visit.  There is an Incentive Plan available that can help to reduce your office visits by 1/2 the weekly amount and would allow you to receive your supplements at a 10% discount.  Information regarding the Incentive plan will be discussed at your initial consultation.
